2. Download and fill in “Proxy Request Form” and submit. This will be sent to our certification committee and you will be sent additional instructions and/or contact information for an evaluator in your area within 2 weeks.
3. Follow any additional instructions.
4. Contact the evaluator listed and set your date, time and place with them.
5. Contact the Certification Committee and inform them of your set date, time and place. They will respond with approval to do your evaluation.
6. Complete your evaluation.
7. Return all documentation to the address listed within one week.
