Proxy Certification Check List

 Go to PestInspectionDogs.com
 Click the “Proxy Certification” tab
 Fill in “Proxy Request Form” and submit. This will be sent to our certification committee and you
will be sent additional instructions and/or contact information for an evaluator in your area
within 2 weeks.
 Follow any additional instructions
 Contact the evaluator listed and set your date, time and place with them
 Contact the Certification Committee and inform them of your set date, time and place. They will
respond with approval to do your evaluation.
 Complete your evaluation
 Return all documentation to the address listed within one week
 Receive your certificate by mail
